Health status, service needs and factors associated with health care utilization among older long-term care insurance beneficiaries with and without dementia

Abstract
dementia; yet there is limited evidence on the care needs and service use of this vulnerable population. Though it has only been conducted to a limited extent, comprehensive assessment is invaluable for understanding health status and service needs of long-term care beneficiaries with dementia. This study is to examine the relationships between health status, care needs, and service utilizations among long-term care residents with and without dementia. This study compared health status, care needs, and service utilizations of older people with and without dementia in nursing home (NH) and home care (HC) settings. A total of 571 older people were examined using interRAI LTCF and interRAI HC. Two-prat models were adopted. Results showed that of the subjects, 45.70% had dementia, and they were more likely to use NH rather than HC service, compared to those without dementia. The dementia group had a higher number of service needs, measured by CAPs, and behavior problems were also higher than among their counterparts. Several factors, including clinical care needs and service settings, length of stay, were associated with health service utilizations of long-term care beneficiaries. This study presented significant differences in health status and service needs between dementia and non-dementia groups. Dementia patients had high care needs related to functional performance and cognition/mental health. Better quality of dementia care is needed in long-term care settings in Korea.
